Surgical technique for penis lengthening
The penis organ consists of a foremost section outside of the body (the visible part commonly referred to as the ‘penis’) and an internal section. The ratio between the external and internal sections is approximately 50:50.
The penis enlargement procedure involves a small incision (of approximately 3 cm in the pubic hair region) and restructuring of the front internal ligaments of the penis. The rear ligaments are left intact, thus maintaining lengthwise stability.
By inserting the patient’s own tissue sideways, we create a new anchor-point for the foremost section at the site of the actual lengthening using suture techniques and materials specially developed by us (no foreign tissue such as silicone or similar is used), which straightens the internal path of the penis that was previously sloped and bent.
We make use of a special geometrical feature here: By straightening the internal path of the penis we produce an extra length, in fact two extra lengths (a slope and a bend into a straight line), which combine to lengthen the penis. What we actually do is alter the ratio between the internal and external sections to enlarge the outer part, your visible ‘penis’.
In doing so, our surgical technique avoids contact with the penis root, the area in which the urethra, vessels and nerves enter into the penis, and leaves these unchanged.
The use of special materials and modified suture techniques ensures that any retraction forces from scarring that could influence the results are minimised as much as possible.
No incisions are made in the penis itself, thus avoiding any possible risks this could involve.
The amount that a penis can be lengthened depends on its current angle and length of the symphysis (the anchor-point to the pelvis), as well as the way that the ligaments are structured.
The existing area of skin covering the penis and scrotum is entirely sufficient to encompass the enlarged part of the penis. Our technique therefore avoids the need for skin grafts or tissue transfer.
Post-op care that may be required:
Patients whose tissue tends to scar excessively should wear a post-op stretching device for some time afterwards. You can find out more about the different applications available for your specific case in your consultation.
Surgical technique for penis thickening
The procedure for the thickening (of the shaft) of the penis is performed by us using the patient’s own fat for a so-called autologous fat tissue transfer (not to be confused with simple the fat tissue injections frequently used by inexperienced surgeons).
Strictly speaking, the penis thickening is the most challenging part of a penis enlargement, as many aspects need to be taken into consideration to ensure good, long-term results.
For example, if an inexperienced surgeon were to suck fat out of you then simply inject it under the skin of your penis, the effects would last for 6 months at the most before the body re-absorbed it. Long-lasting penis thickening therefore requires the application of a special procedure.
Using our technique, we first conduct a special procedure to suck living fat tissue from the inside of the thigh. This is then extensively processed and cleansed before an elaborate procedure is used to carefully insert it under the skin of the penis in a way that ensures it heals to leave an even surface.
A specially engineered bandage with decreasing levels of pressure must be worn for a week in order for the results to be successful.
Now and again, patients express concerns over the success of thickening using their own fat. They are worried it may be re-absorbed, having read or heard this somewhere.
We therefore refer again to the vital difference between ‘autologous fat tissue transfer’ (our technique) and simple ‘fat tissue injection’. Those who are interested will find further reading in Krupp + Rennekampff’s standard work, ‘Plastische Chirurgie’ (‘Plastic Surgery’). They refer to the correct use of the patient’s own body fat to thicken penises literally as the ‘gold standard’ (Issue 3 / 2005). Krupp + Rennekampff’s work is considered to be ‘the standard German-language work on plastic surgery’ (Lehmann), or ‘the principal work to date’ (medizin.de).
Anyone claiming that professionally inserted fat tissue will generally be re-absorbed is unaware of modern surgical procedures.
Thickening using the patient’s own body fat is conventionally performed in conjunction with penis lengthening surgery.
Surgical technique for enlargement of the glans penis
To enlarge the glans penis, a specially developed, high-quality natural material is fitted beneath the skin under local anaesthetic using a clearly defined technique that leaves no scars.
The body will gradually replace this material with its own tissue.
This technique enlarges the glans penis significantly, while the sensation of the nerve endings responsible for stimulation that it contains are slightly dulled (reducing the risk of premature ejaculation). The glans penis thereby retains its natural shape and merely becomes larger overall.
An enlargement of the glans penis is a sensible addition to the penis enlargement surgeries described above. However, for medical reasons it cannot be performed until around 6 months later, not simultaneously.
Is a penis enlargement sensible?
How large is an average penis?
According to the EU condom standard, the average European penis measures 16.5 cm when erect, which is consistent with our findings from the past 10 years.
Some places on the Internet list sizes far above or below the average. Some of these sizes are simply made up and, if they specify any sources for their claims, they often stem from other cultures. It may be true, for instance, that Asian men have a shorter penis on average, but this information does not help non-Asian men to estimate the size of their own penis.
The EU condom standard specifies a size with a Europe-wide consensus of 16.5 that can be called the real European average.
Is penis size important?
As we already knew from our experience over the last 15 years and you the patient have surely already suspected, the answer is ‘yes’.
As the news agencies AFP and N-TV reported on 8 April 2013, after the topic of penis size had been a taboo subject for the scientific community for a long time, a serious scientific study was finally conducted on this issue at the University of Ottawa, Canada. There was an unambiguous and very clear result: Women rate men with a larger penis significantly more attractive.
Enlarging a penis with a size that is below average is therefore sensible and has a significant effect on the patient concerned. The clearness of the result of the study of the University of Ottawa even suggests that it is sensible to enlarge an average sized penis. That even surprised us.
Our experience over the last 15 years also clearly shows that the vast majority of patients gain an increased sense of self-worth through penis enlargement. It causes them to have more success in their relationship, during sexual intercourse, in their social life and in their profession.
The size of a penis should only be modified if it can genuinely be defined as small from a medical viewpoint. We firmly refuse to create a ‘monster-sized’ specimen out of an already large penis.
If you are considering penis enlargement surgery, you should imagine a moderate result, not the best result possible. In other words, you should only undergo a surgery if an increase in length of 3 cm would suffice. For while 5, 6 or even 7 cm would no doubt be more enticing, an operation need not necessarily deliver such results in your case.
We are well aware that patients with an extremely small penis (e.g. 9 cm when erect) in particular would like nothing more than a normal-sized penis, but 9 cm plus (for example) 5 cm still only totals 14 cm and, should the result be on the lower end of the scale, then perhaps only 12 or 13 cm. An increase of 50% would be a tremendous improvement, of course, yet the result still would not quite be 16.5 cm.
Please be aware that we can achieve great results but that we can’t work miracles. We can only rarely assist patients whose erections measure less than 11 cm to attain an average-sized penis, but we can bring them considerably nearer to this goal. Patients must be well aware of this in advance.

As STERN reported in its edition from 30 July 2014, only 150 patients patients in Germany have their penis enlargement performed by all of the other around 400 members of the German Society of Plastic, Reconstructive and Aesthetic Surgeons (DGPRÄC). This is a laughably low number and amount of experience with this operation in comparison to our plastic surgeons.

We generally feel that listing patient testimonials on homepages or forums is problematic, as it is not possible for you as a patient to check to make sure that they are the true opinions of the patients. That is why we have not published anything on this for many years.

Risks inherent to surgery
Germany is the world champion in penis enlargement surgery and WE have been the leading centre in it for over ten years.
Patients from all over the world come to us, even those travelling a very long distance from places such as Australia.
They come to us because we offer a safe operation with very good results and the word has gotten around the world about that.
Unfortunately there is now a whole series of inexperienced and sometimes untrustworthy doctors (some even call themselves “experts”) and clinics that would also like to make money in this field. They attempt to take advantage of the fact that some patients believe that anyone performing a penis enlargement would perform good work at low risk no matter who it is, particularly in Germany. This is not the case!
A penis enlargement operation can involve significant risks if it is performed according to an old surgical technique, by individuals with little experience and personal qualifications, or using cheap materials. It is therefore very important and right that there are reports in the press and on the internet from time to time warning of the risks of a penis enlargement operation. We can only support this, as it serves to protect patients.
As a patient, it is particularly important to understand that there are great differences in the risks and results between the surgeons performing this operation and naturally not every doctor is automatically in the position to perform a penis enlargement operation with good results. Even a chief surgeon in a large clinic or a doctor from a famous clinic cannot necessarily perform a penis enlargement operation. Titles and names do not help to provide quality assurance in this area.
Again and again, patients unfortunately fall for cheap offers on price-comparison sites or in clinic advertisements and actually believe that these places perform the same high-quality work that we do. How is that supposed to work?!
The patient forums on the internet are full of posts from disappointed patients who have obtained nasty results or are otherwise suffering from significant side effects because they wanted to get it done more cheaply or trusted in the wrong doctor. There are no cheap deals for high-quality results and low risk in medicine!
Penis enlargement surgery is a medical treatment like many others. And just as there are a few experienced clinics that perform intervertebral disc surgery with a low risk and excellent results, there are also clinic that do a catastrophically poor job with intervertebral disc surgery.
The same is true for penis enlargements.
So you don’t need to have any fear about penis enlargement surgery but you must absolutely make sure to contact a trustworthy surgeon with many years of experience. Such as us, for example. We have performed the most penis enlargement surgeries by far out of all other clinics in Germany and are also very familiar with difficult and unusual cases.
We take our time to calmly explain the surgical risks that apply specifically to you in an extensive consultation with you before your operation.
Patient safety is our highest priority!
That is why we DO NOT USE ANY solid or liquid fills or implants, for example. Just recently (10/2015) at the annual plastic surgery conference Jahreskongress Plastische Chirurgie it was once again proven by studies that:
- liquid fillers (silicone, paraffin, etc.) can lead to gruesome disfigurations
- the use of artificial tissue matrices (such as Alloderm) for penis enlargements is prohibited, even by the manufacturer
- solid implants (silicone, Elist, matrix, etc.) can cause severe injuries, capsular fibrosis with drastic shrinking of the penis and serious infections
The history of penis enlargement surgery
The idea of penis enlargement is certainly nothing new. On the contrary, it has been around since the beginning of mankind. Evidence of this has even been found preserved in the ruins of the ancient Roman city of Pompeii.
Serious attempts to actually achieve these effects are relatively new (starting around 50 years ago). This may be due to the rapid social changes following the 2nd World War.
American anthropologist Helen Fisher described the development of sexuality in western countries since 1950 as follows: ‘…freedom makes people thirsty and, as a result, increases the demands placed on partners and oneself – all the more since women have increasingly demanded the right to self-determination…’
Penis enlargement operations have always been rather clandestine, even though tens of thousands of men have been successfully treated worldwide in the meantime.
Aesthetic genital surgery for men has yet to gain widespread social acceptance, even though it is capable of having a significant positive influence on the personalities and sexual sensibilities of patients with little effort and should therefore be given a more prominent role in aesthetic surgery.
Penis enlargement surgery is a simple procedure in theory, yet its success depends upon attention to detail and extensive practical experience, as is so often the case. While experienced soft tissue surgeons can almost always produce satisfactory results, failed procedures are unfortunately all too common among inexperienced practitioners.
Cases we will not treat
We will not refuse to assist any patient. However, there are two groups of patients whose treatment by us requires certain pre-requisites:
- Patients who have had previous surgery by other surgeons (failed enlargement surgery by other surgeons).
- Users of stretching devices for penis enlargement.
Explanation of point 1.
We are extremely disinterested in performing corrective surgery on patients who have already undergone surgery by other surgeons.
Our aim is to the best results to as many patients as possible and we give our best for this every day. Moreover, the outstanding results we achieve with our surgeries are in effect our international showpiece and therefore important for our future development.
We can only provide optimal care and outstanding results if the initial operation is performed by us on an ‘untouched’ site, so to speak, so that all procedural possibilities are still available. The scarring and loss of tissue resulting from previous operations performed by other surgeons usually make it impossible to produce results which we feel are completely satisfying.
We provide our patients with intensive and conscientious consultations and are always ready to listen to and answer any of their questions. Furthermore, we calculate final fees for our surgeries that are extremely fair in comparison with international competitors. This is especially pertinent when our experience and the results of our services are taken into consideration.
If patients nevertheless still do not fully trust us and believe that they can obtain a surgery/treatment of the same quality as ours for a lower price somewhere else then they will also have to deal with the possible consequences themselves.
We do not wish to refuse assistance but the more extensive work required for the procedure to correct bad results from previous surgeries will be significantly more expensive (around EUR 11,500).
Explanation of point 2.
We have always had patients who have unsuccessfully tried various ‘quick fixes’ for penis enlargement before they came to us for surgery; in that respect one might expect it not to make a difference to us, as they end up coming to us in the end anyway. HOWEVER:
Several users of penis stretching devices have reported suffering a significant weakening of their erection following or even during the use of such devices. In fact, Doppler sonography tests on these patients revealed extended AA and AV shunts, meaning bridging between blood vessels that alter the blood flow. Such shunt occur relatively frequently and are generally congenital. The shunts seem become distended due to the long-term use of these stretching devices, which constantly compress the penile distal vessels, resulting in a permanent change in the blood flow. These shunts are difficult to control and have an influence on the results of our surgery.
The expansion of tissue caused by penis stretching devices, which gradually decreases, also impedes our surgery, even if only for a specific time.
Therefore, if you do not put your trust in use from the very beginning and believe that you will be able to achieve the same quality of results for a few euros using a stretching device then that is entirely up to you. However, your decision is simultaneously implying that we are somehow shamelessly offering expensive surgery even though penis enlargement would somehow be possible for a few euros. In that sense we are not pleased with your decision.
You must therefore stand by your decision and deal with the resulting consequences on your own.
We do not enjoy performing surgery on patients who have previously used a stretching device for penis enlargement. Such patients must wait 3 months from the last use of the device before undergoing surgery!
In conclusion on points 1 + 2:
We do not wish to pressure you in any way to be treated by us. It is entirely up to you which treatment you choose. However, if you are considering undergoing an operation, we feel it is our duty to inform you that we set certain rules and prerequisites for our work.
In our opinion, this information is just as necessary for a fair and comprehensive consultation as outlining the medical facts. Both are intended to enable you to make a competent decision based on the details at hand.
As you know, we have always performed penis lengthening and penis thickening in one single procedure. This does not go without saying. We would earn far more by offering the two procedures separately, but the combination provides much better results for patients and is also less expensive. The combination saves you around EUR 3,100 overall.
The cost of the combined procedure including materials and all ancillary costs is around EUR 9,900.
(We are pleased NOT to have to raise the de facto price of this operation for our patients in 2014. The difference in price from the old price is merely inflationary adjustment from the start of 2010 (approx. 4 x 1.9 % annual inflation)).

For medical reasons, thickening of the glans penis unfortunately cannot be performed together with the main surgery and costs around EUR 3,600.
The lengthening alone costs EUR 7,200, while the thickening alone costs around EUR 6,000.
On pricing in medical practise in general:
It is always unpleasant for us doctors to have to remind people that surgeries cost money. We doctors are actually here to provide healing and treatment, not for billing.
Yet high-quality medical care comes at a price. This is not because the doctor performing the surgery would like to earn an extraordinary amount, but rather because it involves a significant amount of effort to perform high-quality surgery under the safest conditions possible.
Aesthetic surgeries such as penis enlargement are not expensive in comparison to other procedures but quite the contrary, as they are minor procedures. There are other surgeries required due to disease that cost several times this amount. An intervertebral disc operation, for example, can cost around EUR 60,000. This means that you could obtain 14 entire fat sucking operations for the cost of a single intervertebral disc operation, for example.
The unfortunate part for patients is only that their health insurance will not cover aesthetic surgeries such as penis enlargement, meaning that the patient will have to personally bear the costs. That is why we provide the option of financing procedures starting from 3.9%.
We could, of course, reduce the costs of our treatment by carrying out the operations in centres other than those certified in performing the surgery, by employing under-qualified staff, by using cheap materials, by sacrificing our high level of hygiene, or by having the surgery performed by a less expensive yet inexperienced medical assistant. However, this would be undesirable to both you as well as us! We have been known for many years for performing high-quality work and doing everything we can to ensure the safety of our patients. Your health and good results are more important to us than all other factors.
We are well aware that some patients must save their money for a long period of time or must obtain financing to be able to afford this procedure. Because of this, we are especially pleased with seeing the satisfaction and happiness in the eyes of our patients after the surgery once they have seen that all of their efforts have been worth it and all of their worries and fears are a thing of the past.
